What is GutBusters?

What is GutBusters?

GutBusters is an evidence based, multi-component health, fitness and weight management lifestyle service for men. 

It is based on the latest National Institute for Health and Care Excellence (NICE) PH53 guidelines for overweight or obese adults (click HERE for web link to guidelines)

GutBusters has been created to support, and meet the needs of overweight and obese men in Gloucestershire who have experienced issues which may have prevented them from losing weight, or keeping excess weight off.

GutBusters aims to give men the camaraderie, support structure, tools and knowledge to allow them to make informed decisions about their lifestyle choices (such as diet, exercise and weight management) to help them improve their health, get fitter, look and feel better and to achieve and maintain a healthy body weight.

It does this by offering a range of interconnecting services (e.g. lifestyle health checks, informational workshops and exercise classes) that are linked to either a 4 or 12 week weight management and fitness programme.

The 12 week weight management & fitness programme is similar to the GP weight management on referral scheme, but meets additional NICE PH53 recommendations as it also includes a behaviour change and informational component, as well as an exercise class within each weekly session.

Who runs GutBusters?

The GutBusters service has been created and developed by two local men, Ian Preston and James Powell.  Together, they have over 30 years of combined work and experience within the NHS, health and fitness sector.

As well as being qualified fitness instructors, both Ian and James hold the Level 3 qualifications in Exercise Referral, and the exercise specialist Level 4 Obesity and Diabetes Certificate. This allows them to work closely with clinicians and health professionals within the medical community to deliver weight management and/or exercise programmes for people who are overweight or obese, or have a long term conditions such as type 2 diabetes.

Information about GutBusters Services

 

How the services work

GutBusters has a range of interlinked activities that can be broken down into two main categories of Packages and Services.   More information on these can be found by clicking on tabs to the right.

GutBusters Packages
GutBusters packages are a range of interlinking programmes that support a person through all aspects of their weight management pathway.  They are aimed at giving someone the support, knowledge and skill’s to be able to make informed choices to lose weight and maintain healthier lifestyle choices.  These include:

  • 4 and 12 Week Weight Loss & Fitness Programme’s
  • Bespoke exercise classes and personal training
  • Ongoing monthly maintenance programmes

The packages can be combined and configured to fit an individuals lifestyle and help them more fully to achieve their goals.  For example, someone on the 12 week Weight Loss & Fitness programme may also want to do additional exercise classes, then go onto the monthly maintenance for two months to help them establish lifestyle changes made.

GutBusters Services
GutBusters services aim to promote health awareness and provide accurate information about a range of relevant topics such as how to effectively manage their weight, maintain a healthy body weight and how to improve their health related fitness. These are done via:

  • Face to face 10-point lifestyle health checks
  • Informational workshops and seminars
  • Workplace health promotion

4 & 12 Week Weight Loss & Fitness Programmes

GutBusters currently delivers a 4 week and a 12 week weight management programmes, which are based on NHS guidelines to help individuals to become healthier, lose weight, be fitter, maintain a healthy body weight. They also provide information on a wide range of related topics (e.g. eating healthily and how to exercise effectively) to allow individual’s to be able to make informed lifestyle choices.

Both the 4 and 12 week programmes incorporate the following components within each weekly session:

  • A behavioural change element designed to help men make long term changes to their lifestyle behaviour to help them lose weight and become more physically active.
  • An exercise class designed to improve health related fitness components such as muscular strength, cardiovascular fitness, flexibility and muscular endurance.
  • A themed educational / information component on relevant health topics such as how to lose weight, alcohol, carbohydrates, fat, protein, smoking, stress, healthy eating, physical activity, exercise types, and how to maintain a healthy body weight.

The main difference between the programmes is that the 12 week is able to explore more topics in a greater depth, and includes a personal training session.

How to refer someone to GutBusters

 

Important referral information

GutBusters is able to take referrals for men with certain co-morbidities (e.g. are pre-diabetic or have type 2 diabetes). 

Below and to the right are a list of inclusion and exclusion criteria for individuals being referred into the service.

Referral Inclusion Criteria

  • Aged 18 years and older
  • Have a BMI of at least 27.5
  • Does not have any of the conditions identified in the referral exclusion criteria
  • Has expressed a willingness and motivation to change and lose weight
  • Has given consent and is aware of, and understands the expectations and costs of the programme
  • Is able to attend all of the weekly meetings

Referral Exclusion Criteria

Individuals should not be referred to the service if they have:

  • A significant co-morbidity or complex needs as identified by their GP or other healthcare professionals
  • A BMI of less than 27.5
  • An eating disorder
  • Uncontrolled Hypertension: >180/100mHg (in either)
  • Orthostatic hypotension (also known as postural hypotension)
  • Acute uncontrolled psychiatric illness
  • Acute infections active foot ulceration
  • Any recent myocardial infarction or any other acute cardiac event, unstable angina, new or uncontrolled arrhythmias, resting or uncontrolled tachycardia
  • Any other underlying medical condition that requires more intensive clinical management

Referrals to GutBusters

There are a number of ways to refer or signpost an individual to GutBusters. These are:

  • By completing and sending the online referral form on the right of this page.
  • By telephone: You can contact GutBusters on 07306 334222 or 01452 341773 and make a verbal referral.
  • If you feel that your patient / client is sufficiently motivated, you can signposting them to the service by giving them relevant contact information (e.g. a leaflet, telephone number or email) to allow them to self refer into the service.
 
Important information if using the online referral form:

The online form has two main sections, information about you (the referrer), and information about the person you are referring (the patient /client). Please be aware that:

  • Sections with an asterisk (*) must be completed
  • As the referrer, you need to give referrer consent for us to accept the referral
  • The client must also have given consent before we are able to accept the referral
  • While the client’s choice, it may be beneficial if permission is given for the referrer to be kept informed of the client’s progress.

Why a men’s only service?

 

Rationale for a men’s weight loss service

It’s well documented that being overweight or obese can increase the risk of developing many serious illnesses such type 2 diabetes, cardiovascular diseases, certain types of cancers and premature death. 
 
However, despite more men than women being overweight or obese, men are much less likely to want to take part in, and attend weight loss programmes.  Typically, only around 10 to 30% of those in weight-loss programmes are men, and some weight-loss programmes attract almost no men (irrespective of whether those programmes are delivered by the NHS or by commercial providers).
 
Men tend to use different strategies than women when attempting to lose weight, but are often less likely than women to attempt to lose weight or take part in organised weight management programmes. Some of these reasons why men are reluctant to attend existing weight loss programmes may be:
 
  • That overweight men tend to be less aware than women of their overweight status, and associate increased body size with muscularity and masculinity
  • They have misperceptions around dietary behaviours required to lose weight
  • That they perceive dieting and existing organised weight loss programmes to be feminised (especially as they are typically dominated by females)
  • That they are more attracted to programmes that focus on physical activity as well as diet
  • That they often express a desire to be in the company of others they feel they can identify with
It would therefore seem that there is a gap within weight management services for gender specific programmes that men can identify with.   Such programmes may be more likely to succeed if they appeal to men, motivates them, provides camaraderie and gives them the knowledge and practical skills needed for them to lose weight, become fitter and maintain a healthy body weight.
 
GutBusters aims to fill that gap and provide men with a service to allow them to achieve weight loss, improved health, increased fitness and be able to maintain a healthy body weight.
 

 The Equality Act 2010

The Equality Act 2010 requires that providers of public services need to eliminate unlawful discrimination (including indirect discrimination) and advance equality of opportunity between the sexes.  Gender is a protected characteristic under the Equality Act,
 
However, while weight management services should ideally be universal and proportionately represent and reach all members (and genders) of society.  In reality, these services may not meet the health needs and expectations of overweight and obese men, which in turn, can create an inequality due to the disproportional low uptake of  weight management services by men.
 
NHS and Clinical Commissioning Groups should therefore be at least aiming to equalise the proportional numbers of men and women on publicly funded weight loss programmes. 
 

Therefore, a men’s specific weight management programme (such as GutBusters) has more potential to appeal to men, while helping to reduce any potential inequalities created from under representation in traditional weight management services. 

 
 

What’s the local story?

Currently, there is limited data available about what local men think about things such as their weight, fitness, weight management programmes, physical activity and exercise as a weight loss tool or other experiences or issues affecting them from getting to and maintaining a healthy body weight.

GutBusters has conducted an survey with over 100 local men to ask them about their experiences, beliefs and attitudes around their weight, fitness levels and weight management programmes. While a small sample size and only qualitative in nature, it indicates what local men think and want to help them lose weight, keep a healthy body weight and be fitter. Some of the key findings from the survey can be seen below.

Men’s feelings around their weight & fitness

  • 90% of men had negative feelings about their current fitness & physical health
  • Two thirds of men expressed negative feelings about their weight

What men feel about weight management programmes

  • Nearly three quarters (70%) of men said they would NOT join a weight loss programme such as Slimming World or Weight Watchers
  • Over three quarters (77%) of men said the would join a men’s only weight loss programme to lose weight
  • Nearly one in five men (19%) thought that weight loss programmes are too feminine and are not aimed at men

Men’s feelings about managing their weight

  • 66% of men had tried to lose weight before
  • Three out of four men would like to lose weight or manage their weight

What motivates men to be at a healthy weight?

  • Looking and feeling better, being more attractive and having a better physique was the main motivation for a third of men surveyed
  • Staying healthy, fit & functional, getting advice from a GP and being able to do everyday tasks more easily would motivate 45% of men to be at a healthy weight

What would men like to see in a weight management programme?

  • The top three things men would like to see in a weight management programme are:
  1. Has an exercise component / session in it (34%)
  2. To gain more knowledge and gain new skills (e.g. about exercise, nutrition and how to cook) (31%)
  3. How to deal with mental health & emotional wellbeing issues such as stress (11%)

What motivates men to join a weight management programme?

  • 28% said they would be motivated to join a weight management programme if it was geared towards men, had camaraderie and was friendly & fun
  • Having access to exercise (13%) was the second highest individual motivation factor for men
